Membership Registration

Please read the instructions below carefully for a guide on how to fill the form and then download, fill and submit the forms with the neccesary documents as required.

To download the registration forms:

Right click on the required file and select "Save Target As" to transfer the file to your computer Or click "Open" to display on screen , then click File/Print.

Note:You must download, fill and submit all the three forms together :

For Members => => Member Registration Form (Form M)

For Institution / Head Of Department => => Head (Form M1)

For Technologist=> => Technologist (Form M2)

MEMBERSHIP REGISTRATION GUIDE

1.Read through the structure of the organization first from the MEMBERSHIP page
2.Download the forms, fill and submit with N2,000:00k in bank draft (non-refundable) in favour of The Registrar,    Nigeria Institute of Science Laboratory Technology, Ibadan or payments can be made personally at our Secretariat.
3.Fill the form and the attachments
a..Form M is to be completed by the applicant
b..Form M1 is to be completed by the head of your institute or department
c..Form M2 is to be completed by a technologist who must be a financially up to date and current member of the   institute. Also such person should be able to produce current receipts(s) issued by NISLT Secretariat for at least 3   years annual dues
4.Enclose appropriate credentials (HND/Final Diploma, ND/Intermediate result, SSCE/GCE result, Marriage    certificate if married, 3 passport photographs)
5.After receipt of your form you'll be sent a letter stating if qualified for registration and the fees involved.
6.Payment could be made by bank draft addressed to the Registrar, NISLT, Samonda, Ibadan or be made to account    number: 1812040000471, First Bank, Agodi, Ibadan.
7.After payment, the original teller/bank draft should be sent to NISLT Secretariat together with the forms and    credentials.
8.After the processing, approval letter, Annual Licence to Practice and annual licence to practice will be issued to you.

Payment may be made by direct lodgment to the bank account from any part of the country and forward the original teller to NISLT for collection of receipt.
